I have only three things on this page. I have it Remember repeat customers, then I typed in a Shipping Message. The Shipping Message shows up on the very first cart page when you click to Check Out.

Then the last entry on this page is the Payments Message. On the cart, after you enter personal information and shipping costs, you hit the Go To Payments button. Here you would select the Payments Method, and I only list PayPal. This Payments Message is displayed on this page. Then:
LANGUAGE, COUNTRY and CURRENCY

Here I set the default Message to indicate that the prices are in US dollars. Then I set the countries to US and Canada. Then I enabeled a LIST OF STATES to include the US and Canada. Customers will pick their state, and in my case, including Canadian Provinces. Then:
Submit Changes
As Usual, don't forget to scroll to the bottom of each page and click on the Submit Changes button.
